Homework – Evaluate the survey provided to measure the attitude of hospital employees regarding patient safety.

As feedback, the focus of this assignment needed to be on the evaluation and critique of a research questionnaire used for quantitative data collection.  For the paper submitted, the focus appears to be on the evaluation of study, versus a questionnaire.  As such, no credit earned.  I will accept resubmission.  See attached for my feedback and let me know if you have any questions.
